• Your Medicare card
• Your Private Health Insurance membership information

We are open Monday to Friday (except Public Holidays) between 8am and 5pm.

To cancel an appointment:

Cancellations must be made a minimum of 24 hours prior to your appointment. Please telephone the office during business hours.
